When to Enroll
It's not our annual open enrollment period so enrollment is only if you're newly hired or have gone through a qualified event.
If either of these apply, you have 30 days to enroll in coverage.
Where to Enroll
Enroll through your Paychex account at www.paychexonline.com. Once logged in, click on the Benefits Administration tab. Print the HOW TO guide to help you through each step.

What Benefits We Offer
Before making your benefit elections, we recommend that you lay the ground work for making the right health care decision for you and your family. The following are benefit plans available to you.
Who and When Can You Enroll In Benefits?
Who's eligible for our benefits package?
If you're full time and working at least 30 hours per week, you're eligible for our benefits package after your 30th day of employment.
What about dependents? Your spouse or dependent children can be enrolled in our plans regardless of coverage elsewhere. There is, however, an age limit for dependent children and they'll come off our plans at the end of the month of their 26th birthday.
When Can you Enroll?
1. During our Annual Open Enrollment.
2. If You're Newly Hired - Congrats! You'll need to make your initial benefit elections within your first 30 days. After you make your initial elections, you can re-evaluate them every year during our Annual Open Enrollment.
3. If You Experience A Qualifying Life Status Change (QLSC)
- Marriage or Divorce
- Birth or Adoption of a dependent
- Death of a spouse or dependent
- A change in your spouses employment or health insurance that affects you or your family's benefits
- Change in your spouses coverage/enrollment
- You or your spouse (if you're covered under their plan) are taking a leave of absence.
- Change in your hours from full-time to part-time.
If you do not have a Qualifying Life Status Change, your plan elections will be effective January 1st and will remain in effect for the calendar year.
What happens if my coverage ends?
Coverage will end on the date you leave the Company. Bummer. At that point, you will be eligible for and mailed information about COBRA (Consolidated Ominous Budget Act). If COBRA is not for you, go to www.healthcare.gov for information on getting coverage.
Medical Plans
The medical plans we offer our team include (3) point of service (POS) plans through Priority Health. These plans include in-and out-of-network benefits where you will pay less if you choose an in-network provider.
Plan 1: Health Reimbursement Arrangement (HRA)
What is a Health Reimbursement Arrangement (HRA)
Plan 1 - The HRA Basics
On this plan you and the Company share the deductible and you have copays.
If you choose this plan, your deductible is lower than either Health Savings Account option but you will pay more out of your paycheck in premium. This is because the HRA is more expensive for the Company and Priority Health to administer.
What is a deductible? A deductible is a specific amount of money that you must pay before an insurance company will pay a claim.
What is a copay? A set amount ($10, $25, $40, $75, etc.) for services at your doctor's office, urgent care, or pharmacy. The copay amounts do not count towards your deductible level.

What is a Health Savings Account (HSA)
Plan 2 and 3 - The HSA Basics
In these plans, you have a higher deductible. To help pay for those deductible expenses, you can open a health savings account. Money deposited and used for medical purposes from this bank account is tax-free.
If you choose this plan, your deductible is higher than the HRA option but you will pay less out of your paycheck in premium. You should plan to take this premium savings and deposit it into your HSA bank account.
Why should you consider enrolling in an HSA?
- You're saving money on your medical premium
- Your employer is putting money into a savings account for you
- If you don't have medical expenses during the year, the money in your account rolls over and you are able to build a savings account that can benefit you now and in the future
- If you're banking through Macatawa Bank, once you have $2,500 in your account, you can invest that money into 401(k) like accounts so it continues to grow
Employer Contributions
To help you pay for deductible expenses, we are putting money into your bank account. We hope that you also contribute to this bank account and always make smart choices on picking options that are the best care for the best price. See the Rate Detail sheet for specifics on what we contribute to your account.

Health Savings Bank Account
An HSA is two parts, the insurance coverage and the bank account.
We work with Macatawa Bank to help you grow your account. There are no fees and it's easy to manage your dollars at a location or online.
To open an account, go to www.macatawabank.com/HSA. The process is quick and easy as long as you have your SSN, DOB, and Driver's License or other State ID handy. If you run into trouble, you can always contact your HR team and they will help you complete the process.
Important! You'll need to open an account before the Company can give you any money. Please provide HR your Account and Routing Number.
Flexible Spending Accounts (FSA)
The basics of a Flexible Spending Account or FSA is that you're setting aside a pre-determined amount of money to use for medical expenses tax-free. Only $500 of this money rolls over into the next year, so make sure you think about how much you'll use.
Types of Flexible Spending Accounts
How much can you elect?
- Health Care FSA - up to $2,600
- Dependent Care or Day Care FSA - up to $5,000
- Limited Purpose FSA - up to $2,600
Why would you want to enroll in an HSA and an FSA?
If you enroll in an HSA, your ultimate goal is to keep as much money in that HSA bank account as possible. An FSA will help you accomplish this by allowing you to use FSA funds for vision or dental services. Better yet, you have immediate access to your full FSA annual election.
Have Lasik surgery scheduled? You could use FSA dollars to pay for the surgery upfront, get additional tax free money in your paycheck, and keep money in your HSA bank account. That's a Win-Win-Win!
Debit Cards. How do they work?
If you elect a Health Care or Limited Purpose FSA, you will receive a Benny Card in the mail from bswift. This card is pre-loaded with your annual election for you to swipe and go for services. For services anything other than our copay levels ($10, $25, $40, etc), you will need to submit receipts The receipts are submitted to bswift.
You will not receive a debit card if you elect the Dependent Care FSA. You must submit receipts to bswift for your day care costs.
Wellness and Rewards

As part of our program, we offer annual health screenings and want to reward you if you're enrolled in the medical plan and actively working toward managing or improving your personal health.
Reward - $520 in medical insurance premium savings.
Criteria
- Participate in an on-site or at-home Health Screening
- Take an online Health Risk Assesment (HRA)
On-site or at-home Health Screenings are coordinated every year by our wellness partner, Circle Wellness. You will need to participate in the current year's screenings in order to see savings for the following year.
If you're newly hired, you will receive the health reward automatically in your first benefit year.
Priority Health
We have chosen to partner with Priority Health for their award winning customer service, extensive network, and commitment to improving health and lives. Your Health, Your Way -
Create an Online Account with MyHealth
A user account will allow you to view or order additional ID cards, manage and pay for claims, see a doctor 24/7 with MedNow, or find a pharmacy, doctor, or laboratory. There's also a Mobile App that you can download for Apple or Android.
Finding an In-Network Physician has Never Been Easier
No matter where you live, you can find an in-network provider by going to www.priorityhealth.com and clicking Find a Doctor. The tool will ask you to select a zip code and type of physician.
If you live outside of Michigan, it's important to know that Priority Health uses a network called PHCS or Multiplan. Your physician may or may not recognize the Priority Health name but they will recognize the logos on the back of your ID card.
Can't get to the Doctor? Try Virtual Visits with MedNOW
As a Priority Health member, you can get care from the comfort of your cabana, cubicle, gym, or couch. MedNow is 24/7 virtual care delivered by care providers from one of the top 15 health systems in the country.
Dental Insurance
Our dental insurance is through Lincoln Financial and uses the large nationwide dental network, DenteMax.
Vision Insurance
Our vision insurance is through EyeMed and uses a large nationwide network of providers.
Employer Paid Life Insurance
We offer you 100% employer paid Life and AD&D insurance so you can rest assured that if you pass, your loved ones will have something to help during that difficult time. The coverage for this plan is 1x your base annual salary up to $125,000 maximum.
What's AD&D insurance? AD&D insurance means that the benefit will pay twice if you pass by accident or it will pay a benefit if you lose a limb (arm, leg, eye, etc).
There are other features with your plan like a payout if life expectancy is less than 12 months. Talk to your HR team if you have questions about how to best utilize your life insurance.
Voluntary Life Insurance
Depending on your situation, you may need to elect additional life insurance to cover expenses after you pass or you want to elect coverage for your spouse or dependent children. See below for details on what you can elect.
If you're newly hired, it's important to remember that by electing Voluntary Life Insurance, you can receive up to the Guaranteed Issue amounts without answering any medical questions.
If you've been with us longer than 30 days, during our annual open enrollment, you can increase your election TWO INCREMENTS with no medical questions asked.
- Your increments are $10,000 ($20,000 total increase)
- Your spouse's increments are $5,000 ($10,000 total increase)
Voluntary Life Insurance does offer additional features such as portability (you can take it with you if you leave the company) and a payout if your life expectancy is less than 12 months. Please contact HR if you have any questions.
Employer Paid Short- and Long-Term Disability
We offer you 100% employer paid Short - and Long - Term Disability coverage through Lincoln Financial so when life happens outside of work, you can be comfortable knowing that we've got your back. Both plans offer 60% of your base salary up to a limit in the event or your short-term or long-term disability. See below for details.
Short-Term Disability
- Elimination Period (the amount of time you need to be off work due to the disability in order for coverage to start) - 1 day for accident and 8 days for injury.
- Coverage (what you'll receive) - 60% of your base earnings up to $1,000 per week.
Long-Term Disability
- Elimination Period (the amount of time you need to be off work due to disability in order for coverage to start) - 90 days
- Coverage (what you'll receive) - 60% of your base earnings up to $10,000 per month
In most cases, LTD picks up right where STD ends and the stress of paperwork is nearly eliminated as your case will flow seamlessly from STD to LTD.
Allstate Voluntary Benefits
Accident Insurance
Accident insurance is for off the job accidents and pays a lump sum benefit based on a schedule of benefits (outlined below). You can use the money from Allstate to pay for expenses that your medical insurance doesn't cover, such as deductibles or copays.
Does your child play sports? If he or she has a dislocation or a fracture, you could collect up to $6,000 by submitting a claim to Allstate.
Critical Illness Insurance
Critical Illness Insurance pays you a lump sum benefit based on the diagnosis of a covered illness (outlined below). Money you receive can be used to pay for expenses medical insurance does not cover.
Features Include:
- Option of either a $10,000 or $20,000 benefit where your spouse benefit is 50% of your plan
- Automatic coverage for dependent children (50% of your coverage amount)
- Premiums do not increase as you age
- Level benefit until you hit 70.
- 100% portable, meaning that if you leave us, you can take this with you.
- Pre-existing condition exclusion in that only new conditions will be covered.
Hospital Indemnity Plan
Hospital Indemnity insurance pays you a lump sum benefit based on a schedule of benefits (outlined below). Like Accident and Critical Illness insurance, the money you receive from Allstate can be used to pay for expenses medical insurance doesn't cover, such as deductibles and copays.
- First Day Hospital Confinement (one confinement per year) = $1,000
- Daily Hospital Confinement (per day up to 30 days) = $200
Benefits of this plan include confinement due to sickness, injury, or pregnancy and this plan is guaranteed issue, meaning there are no medical questions or testing.
Retirement 401(k) Plan
It's never too early or too late to save for retirement. For your 401(k) plan, we have chosen to work with Principal Financial for its easy to use tools and commitment to your financial future.
Eligibility: You are eligible for the plan on your date of hire and can start contributing after your 30th day of employment. Any company match contributions start when you start contributing and are paid into your account on a per-pay period basis.
Automatic Enrollment: Knowing how important it is to have money when you retire, the plan has an automatic enrollment feature to help you save. After your first 30 days, here’s what will happen:
- You will be enrolled at a pre-tax contribution rate of 6%.
- Your contribution rate will increase every January 1st by 1% each year until you reach a 10% contribution level.
- The money you contribute will be invested in a Lifetime Secure Fund through Principal Financial.
Do you know your Retirement Wellness Score?
As part of its digital experience, Principal Financial Group has an interactive Retirement Wellness Planner to help you easily manage, visualize and plan for your retirement savings goals.
The Retirement Wellness Planner includes interactive sliders, intuitive prompts and a real-time savings graph that focuses on monthly income in retirement and how long that income may last. You're able to take immediate action—such as increasing deferrals—within the planner itself.
