Hoarding Case Manager

Job Description

The goal of Catholic Charites' Hoarding Intervention and Treatment Program is to help individuals who have indicators of hoarding disorder to create safe and stable housing. By working along the side of our clients, we help them address the many barriers that come with the disorder. This is the only program of its kind in the nation, offering a comprehensive, holistic approach to addressing hoarding disorder. This innovative program provides support by combining case management, in-home coaching, therapy, and community collaboration to improve clients' lives. We empower individuals to regain control of their living spaces, lives, and finally address the root causes of their hoarding. This is a unique and innovative position is for those who like a challenge, and who want to make a difference in the community. Without our services these individuals don't have anywhere else to turn.    

As a Hoarding Case Manager, you will provide compassionate care and support to clients by addressing hoarding disorder. Give support to clients by collaborating with home coaches and therapists to perform a comprehensive assessment, including providing harm reduction along with assessing all six of the hoarding attributes and developing action plans for each. Participate in leading different groups such as support group, wellness classes, and working with clients- hands on in their homes.   You will also be educating the community on hoarding disorder and providing solutions to this complex disorder.  

Requirements

  • Bachelor's degree in Social Work, Psychology, Sociology, or related field.
  • At least six months experience in a social service or health care setting required; 2+ years preferred
  • Valid driver's license, reliable transportation, and vehicle insurance (liability coverage: $100,000/person and $300,000/accident).

"We listen to our client's stories and life experiences. While we are supporting them, they are teaching us. It's important to remember that loved ones with hoarding disorders are suffering a much deeper emotional experience than we can see on the surface."

We have recognized an unmet need of assistance for those facing the Hoarding Disorder in southeastern Wisconsin. Our goal is to shape the community's response to hoarding. Catholic Charities has developed a holistic model of intervention for improving the lives of individuals with the Hoarding Disorder.

Through years of experience, we have created a logic model to work with those who have Hoarding Disorder. This is a coordinated plan of intervention that maximizes resources to achieve the best result to reduce the hoarding, benefit the household directly affected by hoarding, and assist those affected by the problem while being cost effective in the long-term.

The program uses a team approach. This starts assigning a Case Manager, Home Coach and Therapist that perform a comprehensive assessment at the person’s or family's residence. The initial assessment includes screening for imminent harm to self or others along with assessing all 6 of the hoarding attributes (personal safety, physical health, social health, financial wellbeing, environment, and mental health), followed up with action plans for each one.

Impact Story: Beth's Hopeful Future

One year ago, Beth and her mother turned a new page in their lives. As a loving primary caregiver to her mother, Beth worried deeply about the move from their family home. For many years, Beth suffered with the weight of sadness and depression on her shoulders: she showed signs of Hoarding Disorder.

With Beth's mother living safely in a skilled nursing facility, and Beth established in her new home, she sought help from Catholic Charities Hoarding Intervention and Treatment Program. Her goal is to maintain safety and useful spaces in her new home. Brittney, her home coach, is there for her every step of the way.

"I'm much happier here," said Beth, "Brittney is very good at keeping me on track." Brittney visits Beth each week to coach her, support her, and empower her. "Beth has been here almost a year, and she's doing great," Brittney said. Beth's new home is safe and functional, and she is feeling relieved and determined to continue her hopeful journey. "I'm keeping a spark of joy," she said.
