Mae Diogelu yn gyfrifoldeb i bawb yma yn YUA, ac mae cofnodi unrhyw dystiolaeth bosibl yn hollbwysig. Fel aelodau o staff ysgol, mae ein dyletswydd gofal — a’n rôl fel gweithwyr proffesiynol — yn ymestyn y tu hwnt i oriau ysgol. Os byddwn yn clywed neu’n gweld unrhyw beth y tu allan i’r ysgol a allai fod yn berthnasol i ddiogelu, mae gennym ddyletswydd glir i’w gofnodi’n brydlon ac yn gywir i gefnogi lles a diogelwch ein disgyblion.
Safeguarding is everybody’s responsibility at YUA, and the recording of any potential evidence is crucial. As school staff, our duty of care — and our role as professionals — extends beyond school hours. If we hear or see anything outside school that may be relevant to safeguarding, we have a clear obligation to record it promptly and accurately, ensuring the safety and wellbeing of our pupils at all times.

Mae diogelu yn ganolog i’n gwaith fel ysgol, ac mae’n gyfrifoldeb ar bob aelod o staff. I sicrhau diogelwch a lles ein disgyblion, mae’n hanfodol ein bod yn cofnodi unrhyw bryderon — boed yn sylwadau gan ddisgybl, newidiadau mewn ymddygiad neu ymddangosiad, neu reddf broffesiynol sy’n awgrymu nad yw rhywbeth yn iawn. Gall un arsylwad bach, wedi’i gofnodi’n gywir, fod yn ddarn olaf ein tîm Diogelu ei angen i lunio darlun llawnach o’r hyn sy’n digwydd i ddisgybl neu deulu. Good Practice
Safeguarding is a shared responsibility, and every member of staff plays a vital role in keeping our pupils safe. Recording any potential evidence — no matter how small — is a crucial part of our duty of care. Our professional responsibility extends beyond the school gates and outside of school hours, and it is essential that we record anything we see, hear, or sense that may indicate a pupil is at risk. Sometimes, one member of staff’s observation can be the final piece of a much larger safeguarding jigsaw, helping the Safeguarding team understand what is happening in a pupil’s life.
- Record every concern, even if it feels minor, unclear, or based on a gut instinct that something “isn’t quite right.” Professional curiosity is an essential part of safeguarding.
- Note pupil comments accurately, using their own words wherever possible, without interpretation or assumption.
- Record visible changes in appearance, attitude, hygiene, mood, engagement, or general behaviour — patterns often emerge only when small observations are pieced together.
- Submit all safeguarding logs promptly so that the Safeguarding team can build an up‑to‑date and accurate whole‑school picture.
- Stick to factual, objective information rather than opinion, ensuring records are clear, neutral, and evidence‑based.
- Report and log any safeguarding concern observed outside school, including disclosures, concerning behaviour, or worrying situations witnessed in the community.
- Avoid investigating or probing pupils, and never promise confidentiality — instead, reassure, listen, record, and pass the information on immediately through the correct system.
- Trust your professional judgement; if something causes concern, it is always better to log it than to assume someone else has done so.
- Follow school safeguarding procedures closely, including using Edukey/Provision Map for all safeguarding recordings.
- Seek guidance promptly from the Safeguarding team whenever you feel unsure, uncomfortable, or need support.

Rhannu gwybodaeth i ddiogelu Disgyblion
Mae diogelu disgyblion yn flaenoriaeth lwyr i’r ysgol. Er mwyn sicrhau diogelwch a lles ein disgyblion, mae’n bosibl y bydd angen i’r ysgol rannu gwybodaeth berthnasol gyda gwasanaethau neu asiantaethau eraill pan fo pryderon diogelu yn codi. Gwneir hyn yn ofalus ac yn gyfrifol, gan gadw at ddeddfwriaeth diogelu data a chan rannu dim ond y wybodaeth sy’n angenrheidiol. Mewn rhai sefyllfaoedd, mae gan yr ysgol ddyletswydd gyfreithiol neu broffesiynol i rannu gwybodaeth er mwyn diogelu plentyn neu berson ifanc. Sharing Information to Safeguard Pupils
Safeguarding pupils is a fundamental priority for the school. In order to ensure the safety and wellbeing of our pupils, there may be occasions when the school needs to share relevant information with other services or agencies if safeguarding concerns arise. This is done carefully and responsibly, in line with data protection legislation, and only the necessary information will be shared. In some circumstances, the school has a legal or professional duty to share information in order to protect a child or young person. Wherever appropriate, we will seek to inform parents or carers, but there may be occasions when this is not possible if doing so could place the child at greater risk. Examples of situations where information may need to be shared include:
- When there is concern that a child may be at risk of harm or neglect.
- When there is a disclosure or suspicion of physical, emotional, or sexual abuse.
- When there are serious concerns about a pupil’s mental health or personal safety.
- When the school is working with social services, the police, or health services to support a child.
- When the law or national safeguarding guidance requires the school to share information.
Our aim is always to act in the best interests of the child, working closely with families and professional partners to ensure appropriate support is in place.
MARF
Mae diogelwch a lles ein disgyblion yn flaenoriaeth i ni fel ysgol. Pan fydd aelodau o’r Tîm Diogelu yn credu bod trosedd neu lefel ansicrwydd wedi cyrraedd pwynt lle mae angen cymorth neu ymyrraeth gan asiantaethau allanol, cânt gyflwyno MARF (Multi-Agency Referral Form). Mae’r broses hon yn sicrhau bod unrhyw bryderon difrifol yn cael eu rhannu gyda’r gwasanaethau proffesiynol priodol fel bod disgyblion yn derbyn y gefnogaeth gywir cyn gynted â phosibl.
The safety and wellbeing of our pupils is a top priority. When members of the Safeguarding Team believe that a concern has reached a point where support or intervention from external agencies is required, they will complete a MARF (Multi-Agency Referral Form). This process ensures that serious concerns are shared with the appropriate professional services so that pupils receive the help they need as early as possible.

Individual staff members are not informed when a MARF is made — only the Safeguarding Team is involved. This protects the pupil’s privacy, maintains professional consistency, and ensures we respond in a responsible, confidential, and safe manner.

When a school submits a MARF (Multi-Agency Referral Form) following a disclosure from a pupil, it is done to ensure the safety and wellbeing of the child. While families are informed when a referral is made where appropriate, there are clear safeguarding and legal reasons why schools cannot always share every detail of the information. This is to protect the child, maintain the integrity of safeguarding processes, and allow the relevant agencies to respond appropriately.
Cyfrinachedd / Confidentiality
Mae cyfrinachedd yn hanfodol wrth ddelio â materion diogelu yn yr ysgol. Pan fydd disgybl yn rhannu pryder neu wybodaeth sensitif, mae’n bwysig i staff wrando’n ofalus ac yn ddi-farn, cofnodi’n gywir yr hyn a ddywedwyd, a pheidio â gofyn cwestiynau arweiniol. Dylid trosglwyddo’r wybodaeth yn brydlon i’r tîm diogelu drwy’r system briodol. Ar ôl ei chofnodi, mae’n bwysig cofio bod prosesau diogelu’n gofyn am lefel uchel o gyfrinachedd. Efallai na fydd staff yn cael gwybod am bob cam o ymchwiliad posibl, ond gallant ofyn i aelod o’r tîm diogelu am ddiweddariad. Hyd yn oed os na ellir rhannu rhagor o wybodaeth, mae’n bwysig gwybod bod y wybodaeth a rannwyd wedi bod yn werthfawr ac wedi helpu i sicrhau diogelwch disgyblion.
Confidentiality is essential when dealing with safeguarding matters in school. When a pupil shares a concern or sensitive information, it is important that staff listen carefully and without judgement, record accurately what has been said, and avoid asking leading questions. The information should then be passed on promptly to the safeguarding team through the appropriate system. Once logged, it is important to understand that safeguarding processes require a high level of confidentiality. Staff may not be informed of every stage of a potential investigation, but they are of course welcome to ask a member of the safeguarding team for an update. Even if no further information can be shared, it is important to know that the information provided was valuable and has helped support the safety and wellbeing of pupils.
